MATCH Program is a streamlined system of health that improves access to care, engages the patient in their physical, behavioral and social health and utilizes community resources in the best way possible.

MATCH can also help you walk through the rules regarding health insurance in the Healthcare Marketplace - helping you find out the best coverage, and help you apply for subsidies.

Your First Appointment

At your first appointment, you will sit down one-on-one with an enrollment specialist to discuss your current needs. Your initial appointment on average will last one hour. Your enrollment specialist will gather all required documents from you and provide you with an "Enrollment Letter" to provide your primary care physician. As a MATCH client, you will receive 1 year of primary care visits for free at an assigned primary care office. Your enrollment specialist will begin connecting you with resources in the community that you may need.

Case Management

Your enrollment specialist will inform you if you're assigned a case manager. If you are assigned a case manager, you should expect your case manager to follow up with you within 2 weeks after your initial enrollment appointment. Your case manager will provide support and assistance to you with locating resources in the community that can help you improve your overall health and wellness. The role of your case manager is to work closely with you and your healthcare team to set goals and overcome barriers to achieve those patient-centered goals.

Documentation Needed at Enrollment Appointment

MATCH requires the following documentation to enroll in the program**:

  • Proof of Household Income-- if client is working, then one month worth of pay stubs dated in the last 60 days; if patient is receiving unemployment or social security, then proof of income either by statement from bank or government agency.

  • 2016 Tax Return-- if filed.

** Documentation is required at the time of the enrollment appointment

Enrollment Specialist

Casey serves as an Enrollment Specialist for the MATCH Program. Casey grew up in Marion surrounded by the beautiful Blue Ridge Mountains. She followed her dream of helping individuals with special needs and attended Western Carolina University to receive her Bachelors in Communications Sciences and Disorders and Spanish. During her time at WCU, Casey traveled to Costa Rica for 6 months to work with families and children with speech and language disorders. She has played an active role in the National Special Olympics and Farmworker Health Program. Casey says: 

"I would like my community to know that my door is always open, as we are here to alleviate the burden of their challenges, in making linkages to community resources and providing services to live a better quality of life."

MATCH Case Manager

A native of western North Carolina, Donna has lived and worked in McDowell County for over 20 yrs. Donna believes "all people should have the opportunity to live a full and healthy life; MATCH is a wonderful program to help make that happen and I'm excited to be a part of it."

Donna's aim is to support people in reaching their health and wellness goals and to remove obstacles that might hinder them from that achievement.
