Vision

MATCH envisions a McDowell County where all people have equitable access to care and resources that they need to be healthy.

Constituency

Any McDowell County resident in need of access to health care services, local and regional medical providers, and community agencies.

MATCH creates space for all people regardless of gender, race, national origin, religious beliefs, age, marital status, disability, or sexual orientation.

MATCH addresses a critical need in McDowell County for access to whole-person, coordinated care for low income residents who cannot pay traditional health care fees or insurance premiums. Recognizing that access to care alone does not guarantee a patient’s needs are met, MATCH is a uniquely designed model developed to connect patients to additional resources and provide on-going support while enrollees reach their personally-defined goals.

Outcomes of MATCH’s work include:

  1. Improved individual physical, behavioral, and social health.
  2. Decreased number of uninsured people in McDowell County.
  3. Decreased Emergency Room utilization through improved preventative care.
  4. Strengthened communication channels between medical providers, community agencies, and patients.
